Services
Doctor's will often send you for x-rays, try medications and schedule return visits. If you don't get better you will be sent for an MRI or to a specialist who will order an MRI and try different medication. Somewhere along the way or if you don't get better you will be sent to physical therapy and get better.
We are looking for a fulltime Physical Therapist to join our progressive orthopedic physical therapy practice stressing direct access. We have one opening in our Hebron office. We also have part-time and per diem opportunities available in both offices. Only apply if your goal is to provide the same treatment you would expect to receive or would provide your family.
Performance Physical Therapy accepts Medicare, most major insurance plans and private pay clients. Performance Physical Therapy administrative staff will check your benefits and explain your situation before your first appointment. Most insurance companies provide the same coverage whether you self-refer or obtain a referral from your primary care provider.
We specialize in helping people avoid medications, injections, and surgery they may not need. We also specialize in the treatment and rehabilitation of people that have to have orthopedic surgery. This includes all types of shoulder, arm, and hand surgery. Also all types hip, knee, ankle, and foot surgery.
This site offers consumers information, patient stories, and videos on a variety of therapy-related topics. ASHT provides this section of their website to offer patients and the general public information on hand therapy and common hand conditions. This "Did You Know?" site offers consumers information on a variety of physical health-related topics from chronic back pain to exercise and much, much more.

In these times when you find a quality service that makes a difference, I think it is important to acknowledge that business.
I have used Chris DiPasquale's physical therapy skills twice in the last year. Not only has he been spot on with the diagnosis, but has rendered superb and caring therapy. Both shoulder and back issues have been successfully resolved, and strengthening exercises continue to be employed to stave off recurrence.
Hebron's medical services have been growing over the past several years.
Chris and his team are certainly a welcome part of this community.

I've had two full hip replacements and have used Performance Physical Therapy each time. Through their professionalism and individual effort they tailored a program that allowed me to get the most from my recovery in the quickest time. I can't say enough about Chris and Kristen, and would highly recommend them to anyone recovering from knee or hip replacement.

Awesome experience every time I go. Chris is serious about his PT! And you must be too! Always spot on with diagnosis & treatment. All of the staff are polite, accomodating and cheerful. Love the music played! It gets you motivated! I would highly suggest Performance Physical Therapy to anyone looking for a recommendation.

I cannot speak highly enough of how wonderful PPTSM is and has been over the years. I have seen Chris DePasquale for multiple injuries over the last 15 years and he always has me up and going in to time. From a finger injury, to runners knee, to tendinitis to ankle surgeries, (yes, I’m clumsy), and even just a stiff/sore neck, Chris has had had me up and going with ZERO recurring injuries. I strongly encourage anyone to visit Chris before heading to an OR. Don’t underestimate the power of PT! Chris and his staff are extremely accommodating and flexible and the environment is welcoming.
