Services
Please arrive 10-15 minutes prior to your appointment to fill out the proper paperwork. If possible, please fill out the forms prior to your appointment and bring them with you. Also, bring your insurance information and doctor referral with you (if applicable). Bring in any additional reports as in X-rays, MRI, etc or doctors notes that you may already have.
While the state of Alaska does not require prescriptions from a physician to receive physical therapy, it is highly recommended to obtain a prescription if one is planning on using medical insurance. The majority of insurances require a referral from a physician in order for physical therapy services to be covered.
Our job at Arctic Physical Therapy is to help improve the lives of others, but truly our patients are the ones that enrich and inspire us on a daily basis. As a thank you to those that we serve, every fall we will be offering an annual "Scholarship for Hope." This scholarship will be given to an individual who has overcome enormous challenges and obstacles with grace, courage, and hard work.
