Failed Communication. Not one care provider that visits the house knows what the other is doing. Sad. I called intake to tell them patient was transported back to the hospital for sepsis. Wound care stayed they spoke to wife and reported possible infection to wound. Untrue I was not home. Renter to the hospital is a fail for home health. Don’t leave your partner that is receiving home care alone for any amount if time. Praying for my husband to not have his leg amputated.
