PMH Wellness opened it's doors to address the need of individuals affected by life stress, brain based mental health conditions and an overall dissatisfaction with life. We see a need to improve the quality of life by addressing the root cause of your concerns. We work closely with our clients to develop a practical treatment plan that's easy to follow.
A psychiatric evaluation involves obtaining an in-depth and detailed history of experiences in the areas of physical medical health, psychological health, behavior health, spiritual health, social health, financial health, and family history. In addition to history taking, laboratory studies are recommended.
Payment is due at the time of service. Acceptable forms of payment are: All major credit cards (American Express, Mastercard, Visa, Discover), most debit cards with Mastercard/Visa logo, and cash. An in-depth and detailed history of your medical, psychiatric, behavioral, social, spiritual, psychological, financial history and more.
