Will Send you an email with all your initial paperwork through Icanotes. Please have the paperwork completed prior to our scheduled appointment so I can be prepared and get to know you a little. If you have a chronic medical issues, please attach your most recent physical or any important notes your doctor believes I need to know to better assist you.
Co-pay required at time of service. Annual deductibles with your insurance company are payable at time of service. You will be billed for longer sessions and will be responsible if your insurance company does not cover the overage. 6 prepaid sessions offered at @ $100.00/session (for a 55 minute session) a total of $600.00.
