You do not need a physician referral to receive care. New to our clinic? Welcome! Kindly arrive 15-20 minutes before your scheduled appointment to complete the new patient registration. All other major health plans are accepted.

We have been a part of Linden community for decades. We are conveniently located at the corner of Cleveland and Republic Avenue (north of Cleveland/Hudson Avenue intersection). We are a primary care clinic. We provide adult, geriatric (senior) and adolescent care. We offer women's health and wellness programs, annual physicals, team sport physicals, among other services.
