In order to keep our costs low and maintain a fee schedule that is affordable to as many patients as possible, this office exclusively accepts private pay. This means the patient (or the patient's guardian) will be solely responsible for the cost of care at the time of services rendered.
We do NOT accept any third-party payments from the patient's insurance, worker's comp, or Medicaid, nor do we provide a superbill for self-submission to aforementioned insurance entities.We, however, CAN accept payments from Medicare or your HSA bank card at the time of services rendered and provide you the basic receipt you would need for your required tax records associated with your HSA.

Activator Methods Chiropractic Technique is a gentle and comfortable approach to providing specific chiropractic adjustments to each patient's unique body. By using a hand-held instrument to deliver the adjustment, the patient does not need to be turned or twisted into various positions for the adjustments.
Whether to vaccinate or not should be a personal choice and we feel it is important to make an informed choice. All information is kept strictly confidential. Completing these forms prior to your first visit greatly assists our staff in creating and updating your file while limiting your overall wait time.
