You can find a provider anywhere. But when you know your provider cares for you and your family, life's little emergencies don't seem so scary. We offer family health care (ages 10 and up) at Care Point Health & Wellness Center, and invite you to contact us if you're looking for a primary care provider.

The Case Manager provides social work and client advocacy services to clients of Big Bend Cares. This position reports to the Director of Client Services. Ensures the completion of the required intake procedures per the Dept. of Health's HIV/AIDS case management standard and guidelines. Assesses need for internal and external support systems.
