With over 200 years of combined experience, our staff of professionals is here to meet the needs of each and every patient and resident. Services
Titusville Rehabilitation & Nursing Center provides coordinated, comprehensive services to meet the care needs of the individual resident and patient. The interdisciplinary team members will collaborate with residents, patients, families, and staff to develop an individualized plan of care for each resident and patient.

We have developed successful clinical systems including specialty programs and best practice programs which enable our nursing teams to achieve enhanced resident satisfaction.Our Administrative staff, including our administrator, business office manager and admission/marketing staff work together to manage the day-to-day operations and business-related activities.
You can ask to see or get an electronic or paper copy of your medical record and other health information we have about you. Ask us how to do this. We will provide a copy or a summary of your health information, usually within 30 days of your request. We may charge a reasonable, cost-based fee. You can ask us to correct health information about you that you think is incorrect or incomplete.
List not all inclusive - inquire today for how we may provide services unique to your situation. The facility is staffed with licensed nurses 24 hours per day and a team of licensed Physical, Occupational, and Speech therapists.

The interdisciplinary team works with the attending physician to develop an aggressive rehabilitation program and ensure the complex medical needs of the patients are addressed and a comprehensive plan of care is developed to help these patients return to their normal level of functioning as quickly as possible.
Certified Nursing Assistants (CNA) are the heart of our general nursing and restorative care programs. They focus on providing care and comfort while striving to enhance the joy and dignity of life. Our restorative CNA assists residents in achieving their highest level of practical functioning, while our psycho-social CNA helps our activity director in presenting an array of social events and recreational activities.

You guys don’t even deserve A one star, The workers in this place does not care about your family or you’re loves one, But only one worker does and her name is ELENA. So please do not send your family members here the workers are Super lazy and doesn’t care at all!! Everything is about money to them!!

My mother was in here for rehab. I've tried many times to call her. There are no resident phones and just one cell phone in the facility. I have to call them (which I often cannot get then to even answer at all) then request they take the phone to mom and them have mom call me. I've waited 20 minutes on hold just to talk to her wing desk. I have to wait until they get around to taking the phone to her to call back. It's frustrating and time consuming. At moms age it is bad for her to have little family contact yet I have often just given up trying to talk to her. There is no voice mail to talk

Easy to get loved ones in. Nice at first, but when family gets to involved and wants to bring a loved ones home they make it as difficult as possible for the transition process up until the last moment. These people that work for this facility are snakes and liars. They are not there to help and rehab. They are there to take money and send you on a wild goose chase. They neglect and steal. Do not leave your loved one here. When it comes to their own made up rules for visitors you must win the lottery. They pick and choose who can have "outside" visits. Just don't do it. Find a different place.

If I could give this place less stars I would. Pathetic to know that other facilities are open to 2 visitors but this abusive place still makes you visit from the outside while staff in the inside isn't even required to wear mask espically around residents that have serious medical issues. Disturbing to see maskless employess in a nursing facility during a pandemic and kidnapping residents and keeping them from family and friends.

So I was terrified about bringing my husband here bc of all the horror stories, but let me tell you this place has been absolutely AMAZING!!! Carrie and Mary Beth made me feel sooooo much better, they are some really kind hearted women right there. Physical, occupation, speech therapy what a freaking team they got there. My husband had a stroke and they said he would never walk again, boy they sure proved them drs wrong, THEY HAD HIM WALKING HIS FIRST DAY!!! Im beyond grateful that he is there. Thanks titusville rehabilitation, you guys ROCJ!!

Called about 9 times regarding my dads clothes he never got back. 2 of them being Christmas gifts he received. They always say they will call back. They never have. I realize it's just clothing, but still it means something to us. This is likely his last year with us. No one seems to care there. Not even a courtesy call from them.

Do not allow your loved one to go to this place. My mother is a amputee of one leg and they let her sit with a black foot on the other leg which resulted in amputation of 3 of her toes. You can never get a hold of anyone and when they take your name and number they don't call you back. I could go on and on, my only suggestion is to make sure your loved one does not go here

My mom needed to go from the great care at Parrish Hospital to a rehab facility after a substantial injury. None of us were very impressed when she arrived at Titusville Rehab. She was very ready to work hard and get home, but that was not what she experienced. The amount of rehab time was not impressive and not fully meeting the amount of time it should have. She had experienced *days* of constipation and this was not addressed. She did not enjoy the food. To compare this, right from the start when she arrived at Sea Pines in Melbourne the difference was night vs day. Literally, the only “complaint”

My brother was admitted last November suffering from the onset of dementia and recent surgery. The staff has taken excellent care of him for nearly a year. They have been committed to his rehab and comfort. Sadly, he will likely die there, but definitely not from neglect.The staff has communicated to us in Wisconsin numerous times for various issues. The aides, nurses, and other professionals have an abundantly difficult job, and they deserve appreciation and empathy. I applaud them for caring for our sick.
