Evolve Physical Therapy was created as a response to the growing trend of declining quality in health care delivery. We are different. Flare ups are not uncommon. If you have a flare up, give us a call and we'll schedule you quickly to see what the problem is. You will be re-evaluated on a monthly basis and when you see your doctor, we will work with you hand in hand until you achieve your goals. We contact your insurance provider on your behalf, verify your benefits and get all the details so you know exactly how much your treatment will cost.

Evolve Physical Therapy is Oregon's best physical therapy clinics treating vertigo and other vestibular conditions that cause imbalance and dizziness. We can provide physical therapy treatment for vertigo at both our Sherwood and Bethany locations. Vestibular Rehabilitation Therapy is the treatment of the symptoms that create a sense of spinning, imbalance or "feeling off" that leads to imbalance or falling.
The injuries that occur with individual and team competition can range from minor to catastrophic. Sports injury treatment can range from education to avoid over-training, bio-mechanical analysis to change technique (e.g., high-speed video analysis for running), or more traditional physical therapy to address pain and swelling reduction.

I’m so grateful I called Evolve Physical Therapy! I’ve made amazing progress in my recovery thanks to JP aka Josh Pate. He developed a plan to help me regain my strength and mobility. I had so much back pain when I started 2 months ago and it’s so much better! I especially love how he adds my exercises to the MedBridge app. I can login each day and it runs me through my PT routine and keeps track of the time allotted to each. I don’t have to remember each exercise or how many sets.
The entire staff at Evolve have been warm and welcoming. I highly recommend Evolve.

I wholeheartedly recommend Josh “JP” Pate as a physical therapist at Evolve. He was able to quickly drill down to the root issue, develop a comprehensive plan of attack, and execute it. I came to Evolve Physical therapy after suffering for 10 years with vertigo and neck issues. The front desk girls are amazing and scheduling was a breeze. Josh Guyer diagnosed and treated the vertigo in the first visit. He knew exactly what was happening and how to fix it. His knowledge and experience with vertigo made me feel very comfortable from the beginning. I felt a plan was designed specifically for me and for my specific concerns and issues. I highly recommend seeing JP at the Sherwood location. I went in there about 6 weeks ago with left knee pain so bad I wasn’t able to walk correctly. I thought I was going to need another knee surgery. On the first visit I was so impressed with his knowledge of anatomy and how quickly he diagnosed the problem and started working on a solution. I actually felt some pain relief after the first visit. I seen him 2 times a week and followed his workout treatment plan at home. I walked out of there today significantly stronger with no knee pain full flexibility and the knowledge to keep improving my

Evolve PT has been different than any clinic I've been to - in the best way!
JP has taken the time to listen to my concerns and has walked me through what my recovery will look like. I've been navigating shoulder pain for a decade and this is the best help I've gotten. After only 2.5 weeks I've seen so much improvement. I'm finally optimistic about a future without shoulder pain. I didn't think I could ever say that again. I highly recommend JP if you're looking for a comprehensive and thorough path to healing.

I found Evolve over the summer when I suddenly started experiencing vertigo. I'd self-diagnosed as having BPPV (benign paroxysmal positional vertigo). I wasn't sure what was going on but the symptoms I found online seemed to align with what I was experiencing. I searched Yelp for BPPV treatment and there weren't many results, but Evolve appeared and had one very high review that specifically mentioned BPPV, so I reached out to them from their website. I saw Matt on an emergency basis for vestibular therapy for BPPV. Evolve was able to get me an appointment less than an hour after calling whereas the soonest I could get an appointment through my healthcare system was 2 days later. Anyone who's ever had a BPPV episode knows that there's no possible way to endure it for 2 days. I was so grateful Evolve was able to get me in so quickly and even more grateful that I didn't have to explain what BPPV is to the scheduler. She knew what it is and that I had to be seen quickly. I've been dealing with neck and shoulder issues for over 4 years, and have seen so many different physical therapists, and Matt at Evolve is one of the best to ever do it. He is so well-versed in treatments and techniques. Very knowledgeable. But most importantly, he makes therapy fun. Matt, and the entire staff, are so engaging and kind. I've had to cancel appointments last minute because of headaches that've come up, and they are always so understanding. They truly just want what is best for the patient. You can feel the care and concern every time you walk into their office.

I was referred to Evolve by my physician who recommended Matt and Matt because of their expertise in dealing with neck/cervical spine physical therapy. After injuring myself In a yoga class, and than again just getting out of a seat I knew or was pretty sure surgery was in my future. Actually Matt the physical therapist found me at a happy hour spot, and gave me his card. What made Evolve perfect for me was the chance to let the patient\ client help be in the process of the recovery, but more so Matt got to know me a bit, and I think a typical busy, loud gym type atmosphere would not have been ideal. I could have easily stayed at OHSU PT but the one on one support I was, and still am receiving at this location tends to be a better fit, and

Personable, knowledgeable and honest about his expectations and treatment. I was referred to Matt Whittaker at Evole physical therapy from a trainer at the local YMCA. I was having issues with my neck. My range of motion was limited and pain level high. I called for an appt and was worked in that same day. Matt worked with me one on one for an hour. He educated me using his model of a spine and gave exercises to do at home. After 5 sessions im back to normal and can attend my exercise classes. It was worth going out of my plans network to seek help. I highly recommend this practice.

I feel like i've tried every physical therapist in the PDX area before landing at Evolve PT and working with Matt Whitaker. I felt compelled to write a review as I'm *FINALLY* making some progress with my chronic issue! It seemed like I was on an endless loop of recommended PDX PT's who would ultimately stray to the same protocol of endless strengthening exercises, clamshells, lunges, etc which never resolved my issue with my diminished glute / lower back / hip function probably from 20+ years of Triathlon and running marathons.
