PHS Hospice Care
PHS Hospice Care is a life-affirming program that promotes the physical, emotional and spiritual well-being of patients with life-limiting illnesses, along with that of the caregivers and family members of the patients. We are an interdisciplinary team of professionals and volunteers offering supportive services which maximize the patient's comfort, quality of life and autonomy.

PHS Hospice Care is available for assistance 24 hours a day, 7 days a week. The goal of the Hospice program is to help those diagnosed with terminal illness live a full, meaningful life for as long and as painlessly as possible. Care offers many services under our Hospice program including in-home care, medication management, pain management and grief counseling.
Services
PHS Hospices Cares' goal is to provide a comfortable lifestyle through expert symptom management and support for those with terminal illnesses. We work individually with patients and their friends and family to provide a care plan that is tailored to their needs. Our team of medical professionals, staff and volunteers work together with the patient, his or her attending physician, and family to provide hospice care.
Your doctor will continue to manage your care even while you are under the Hospice Benefit. The clinical staff of PHS Hospice Care is specially trained to manage your pain at all times. They work with your doctor to make sure you are comfortable. Scheduled, routine visits from the hospice staff will assist you and educate your loved ones in your care.
Family support does not end with the passing of the hospice patient. Bereavement services are provided by PHS Hospice Care for 13 months following the death of your loved one. The family will receive phone and visit support, along with regular mailings of literature that will support and educate family members as they grieve.
Medicare cover services provided by PHS Care Hospice. The Hospice Benefit for Medicare patients helps cover medication, medical equipment and supplies, co-payments and family support counseling. Be certified by his or her physician as having a "medical prognosis that his or her life expectancy is six months or less, if illness runs its normal course".
