Debra Middleton FNP-BC, ENP-C was awarded The 2019 Outstanding Rural Health Provider Award.

Services
Our physical therapy team works toward specific goals of achieving the resident's highest functional level of independence. Physical Therapy focuses on identification and rehabilitation of individuals with physical impairment and pain, as well as future injury prevention.
Our occupational therapists use task-directed activities in the treatment of our residents whose functional abilities, daily activities, and independence have been impaired by illness, injury, or aging.They assist in allowing our residents to function well in their environment. Speech therapists are able to provide a range of interventions to asses and help restore abilities in the areas of communication, swallowing, and cognition.

Cooperstown Medical Center and Griggs County Care Center are excited to announce our building project! We are anticipating to break ground in the beginning of June, this will be followed by an official groundbreaking ceremony to be held in July. We are currently in the midst of our Capital Campaign raising the funds to continue to provide the essential services for our community.
CMC's Emergency Room is designated a Level 5 Trauma Center. The ER is also equipped with an eEmergency system. This system is a two way video system that allows our ER provider to be linked to Avera-McKennan Hospital in Sioux Falls, SD. This system gives our ER immediate access to emergency trained physicians, nurses and specialists at a moments notice.
Joyology replaces the work Activity at Griggs County Care Center. Through a comprehensive assessment we are able to determine how the optimal level of Joy can be injected in to each residents day. We practice "person centered care" which allows us to tailor to each unique individuals preferences, values, and needs.
Our social service team assists both residents and their families during transitional periods. They are here to support not just the resident but also the family. Our social service team assists from admission through discharge. We also provide essential information, manage requests and concerns, help in care planning and discharge planning for each resident.
