Dr. Flatt is a native of Jackson County and is pleased to serve his community & the Upper Cumberland for several years now. Margaux is a native of Jamestown, TN and has resided in Cookeville since 2008 where she worked as an RN at Cookeville Regional Medical Center. Margaux started at Upper Cumberland Family Physicians in 2014 as a Family Nurse Practitioner.
How to request a prescription refill? Most ongoing prescriptions can be refilled by phone if you have been seen within the last six months. Call our office at least one week before running out & leave the following information with the receptionist: patient name, medication name, dose and pharmacy location.
Payment and/or co-payment are due at the time of service. If we are a participating provider of your insurance, we will file your claim at the time of service. Any remaining balance after we file your insurance is due within 30 days. To make payment arrangements, please contact our office. We accept cash, check, MasterCard and Visa.
