Our new state-of-the-art facility consists of two distinct neighborhoods each boasting large private rooms with full private baths. The neighborhoods include large comfortable lounges and spa rooms. Our residents will enjoy beautiful courtyards and have access to state-of-the-art therapy equipment and an impressive calendar of activities.
Colfax Health & Rehabilitation Center is known for its small, personalized atmosphere. We provide specialized programs in a homelike atmosphere to meet each individual need. Along with nursing, all staff members including housekeeping, dietary, maintenance and administration have special training in dementia care.

We understand you have many options when it comes to choosing a facility for your post hospital stay. Colfax Health & Rehabilitation Center's Transitional Care Unit is equipped to deliver the highest quality rehabilitative care in our beautiful rural setting.
This new complex offers 10 single occupancy and 2 double occupancy apartments that are designed to provide care and support services to residents living independently. Services include meals, medication management, housekeeping/laundry services and assistance with bathing. Each apartment has an emergency call system and staff is available to provide as needed assistance 24 hours per day.
This facility was designed to promote safety and socialization for residents needing supervision. Programming specific to each resident is provided to enhance quality of life and independence. Assistance with activities of daily living is provided and staff is on duty 24 hours a day. This setting is a great fit for individuals that are no longer able to safely live alone at home but do not require the medical care provided in the nursing home.
