Sky Lakes Medical Center ("Sky Lakes") is committed to protecting the privacy of our patients' information. We are in the process of investigating an incident that may involve the loss of some of that information. This notice describes the incident and outlines additional measures that we have implemented to safeguard patient information going forward.
The assessment is part of our local health improvement process that uses data to identify priority issues, develop strategies for action and establish an accountable system with measurable indicators to track trends over time. The Community Health Improvement Plan (CHIP) serves as our guiding document to reaching our shared goal of a healthier Klamath.
The park on the north end of downtown Klamath Falls is another investment by Sky Lakes in the community to encourage more physical activity. A new play area at Kit Carson Park that opened in 2016 adjacent to a residential neighborhood also was funded by the medical center as a gift to the community.
