Peak Performance Sports & Spine is a therapist-owned, outpatient orthopaedic physical therapy private practice. We specialize in orthopaedic and sports injuries, spine injuries, work or car accident injuries, balance problems, dizziness, headaches, joint pain and stiffness. Peak Performance is more than a physical therapy clinic.
We want your appointment to run as smoothly as possible so that we can get you on your way to healthy. For your convenience we have provided a pre-registration form that you can fill out on the site and send directly to us by clicking "submit" or you can download the full intake form, print it out, and bring with you to your appointment.
Wear comfortable clothing that allows free range of motion and in which the physical therapist can assess the injury site / ailment. During your first session, the physical therapist will spend time with you to learn about your condition, limitations, and related health history. The provider will then take specific measurements of the injury / ailment that is causing your problem and put together a treatment plan for you.
The Health Insurance Portability & Accountability Act fo 1996 ("HIPAA") is a federal program that requires that all medical records and other individually identifiable health information used or disclosed by us in any form, whether electronically, on paper, or orally, are kept properly confidential.
