Hello, my name is LaDeidra and I am a Holistic Registered Nurse, Certified Nurse Case Manager, Licensed Massage Therapist, Reflexologist, Traditional Usui Reiki Master (Teacher), Life Purpose Coach, and Spiritual Counselor. I have a passion for helping people relax and heal through holistic therapies.
Please complete the Health Declaration at each visit and the Medical Questionnaire prior to your initial visit. The first 2 forms are NECESSARY for treatment. You also agree to update your Therapist of any changes to your medical history. Do you have any additional comments or questions to be answered before we begin your session?
