Heritage Health Center (HHC) is a non-profit, federally-qualified community health center (FQHC) providing primary care and services to those living in the Big Horn Basin. Heritage Health Center is a primary care clinic with a highly-experienced medical and behavioral health staff including a physician, a physician's assistant, nursing staff trained in care management, two licensed clinical social workers, and a licensed practical counselor specializing in youth counseling.
The clinic offers traditional healthcare services such as routine primary care visits, preventative care, chronic disease management, and lab work, as well as, behavioral health counseling in a team-based model.Heritage Health Center Services are available for all people, including those with insurance and those without insurance.

Services
Whole Health means your family's primary healthcare, behavioral health, patient case management, chronic disease treatment and care, preventative dental, immunizations, sports physicals and lab tests can all be done under the same roof. And these aren't all the services we offer. Do you need assistance with your health insurance?
2 If patient has been diagnosed with Diabetes patient needs to bring most current A1c or their glucometer to show past blood sugars. 3 If patient uses CPAP or BiPAP at night patient needs to bring in data showing compliance. 4 The use of controlled/sedating agents such as narcotics, sleeping medication or anxiety medication.
Heritage Health Center currently does not offer childhood, teen or travel immunizations. However Park County Public Health offers a number of programs for vaccines. The Student Health Center is a collaboration between Heritage Health Center and Park County School District #1. The Heritage Student Health Center provides counseling services during school hours to the students of Park County School District #1, ages Pre-Kindergarten through 12th Grade, and their families.

Heritage Health Center treats anyone regardless of insurance or ability to pay. Patients who do not have insurance or have a high deductible plan, can apply for our Sliding Fee Scale that provides services to patients at discounted rates based on their household size and income.
