On behalf of our entire team, thank you for considering our nursing and rehabilitation center for you or your family member's care. Our facility features 231 skilled nursing beds and includes state of the art equipment to help accelerate the recovery process of our residents. Our wide variety of services enables our patients and residents to receive the medical, restorative and supportive care they need.
After an injury, illness, or surgery, a patient may require specialized attention and therapy to recover. At Park Avenue Healthcare & Wellness Centre, rehabilitative therapy plays a crucial role in helping patients recover, regain strength, or find new ways of doing things that were once second nature.

They have taken really good care of my Mother for almost a year. They call me immediately with any changes, have great recommendations, and I can see the comfort they bring my mom.
Every nurse or CNA I've dealt with has been professional, caring, and honestly did their best to care for my loved one. As another reviewer mentioned, it looks "under construction" there are ceiling panels missing because they're sealing the roof against leaks. No big deal. Doesn't affect the care. I'm very happy with Park Avenue

Wow, staff do go above and beyond, despite the place looking like it is (under construction). My father only stayed there for about 2 weeks. Rehab department and nursing staff were very nice. The two gentlemen in the front office, Kenny and especially John (some people called him JV depending on what he is doing that day I guess- he wears scrubs one day, but wear a button up shirt and a tie for the most part) them two are just as wonderful as the other other staff. Will bring my father there again if he ever needs a nursing home again.

This place lets visitors stay until they want. The problem is the employees. They take to long to help patients. My grandma needs help using the restroom. I had to go and ask for help. One of the employee said, "She choose the wrong time to use the restroom, we are on our lunch". I was furious. They do come and check on her but not when she calls them for help.
