The Community Health Centers of Benton and Linn Counties provide patient-centered primary medical care that focuses on the whole person. We understand how physical health, mental health and oral health all impact each other. We are committed to helping each person achieve these health goals and lead a happier, healthier life.

When you join the Community Health Centers patient care family, you have a team of doctors, nurses, behavioral specialists and others working together, devoted to your care. Each of our six locations is accredited as a patient-centered care home at the highest level. It's a designation we take seriously and you'll notice the difference in our services.
Services
Community Health Centers of Benton and Linn Counties provide a medical home that promotes and supports health and wellness. Offering care that is inclusive and available to anyone regardless of insurance status, economic status, language, age or health status. Working together with patients/consumers/clients to have them involved in decisions and actions to improve their health.
In 2004 Benton County opened the Community Health Centers of Benton and Linn Counties, joining a national network of community health centers providing high quality accessible care to people in need. Federally Qualified Health Centers (FQHC) provide services to address financial, geographic, language and cultural barriers to care.
Our primary care providers and team of health care professionals will treat you with a personal touch at our clinics in Corvallis, Monroe, Alsea, Lebanon, and Sweet Home. Our patient-centered teams are dedicated to providing you with excellent, affordable care. Four of our clinics: Benton, Lincoln, Monroe, and East Linn are recognized by the Oregon Health Authority (OHA) as Five Star Status clinics.
The nation's network of Community, Migrant, Homeless, and Public Housing Health Centers provides high quality, cost-effective, primary and preventive health care to the medically underserved, regardless of insurance status or ability to pay. Over 1,200 health centers serve over 9,000 urban and rural communities in every state and territory.
The Community Health Centers of Benton and Linn Counties have become models of excellence in health care innovation. This success is due in large part to community ownership and direction under a community board - over half of the board members are patients of the Health Center - that is responsive to the community as well as individual patient needs.
