Services
You are responsible for a portion of the cost of care. However, no patient will be denied healthcare because they are unable to pay for their portion of the cost. CHC/SEIA receive a limited amount of federal funds to assist cover at fare cost of your care not covered by public/private insurance. We are committed to providing medical and dental care for all those in need in our community.
The goal of CHC/SEIA is to provide healthcare that focuses on prevention and early identification of common medical problems such as diabetes, high blood pressure, obesity and heart disease. Since 2003, we have provided a medical and dental home to patients that are unable to access health care elsewhere in the community.
Your oral health is our primary focus because it is a key to complete overall health. It is our mission to create a healthy community starting with your family. We take pride in providing exceptional quality service that will leave a long lasting impression. Oral health education and prevention is our approach to keeping you and your family healthy.
Behavioral health providers are available on site or through our telehealth system to address an array of behavioral problems including depression, mood disorders, anxiety, alcohol and drug use for our primary medical patients. The behavioral health providers will work closely with the CHC/SEIA primary care providers who make the referrals to assist our patients address their behavioral health concerns.
Community Health Centers of Southeastern Iowa, Inc. is dedicated in providing the communities we serve with excellence in medical, dental and behavioral health care that is compassionate, affordable and accessible for patients. Patients can apply to receive assistance through our Sliding Fee Program that allows patients to pay for their services based on their household income and family size.

I have been going to CHC in Burlington,IA for over a year and a half now. I can say that I have had all good visits with ALL the staff Ive dealt with from the time I get checked in until I walk out. It's a small clinic with lots to offer! I usually never have to wait long, especially with doing the recommended checking in 15 minutes early. I am seen within 5 minutes! I am also going to take on my dental needs with CHC because I was having to find a new dentist. I believe I will have great care in that department as well. Very clean, professional clinic! I am SO glad I chose CHC for all my medical

Came to the dental for a root canal, supposedly they did a partial and I was to return to finish. Well due to Covid-19, they canceled my next appointment which was understandable but after reopening and scheduling my visit after 3 months, the tooth and filling cracked. I called pleading 2 xs to please move up my appointment, was told no and refused any help at all. Went to another dentist who told me that the tooth and my jaw are infected so badly he couldnt believe I was dealing with that much pain and gave me the antibiotics I asked for from THIS HORRIBLE PLACE!!!!!! Unbelievable that they can

I drove somebody here, so I don't know first hand, but from what I saw, the staff is polite and efficient. The waiting room was quiet and comfortable. It usually takes about an hour from start to finish for a dental appointment. Community Health is fixing the work that Aspen did. Come here. Don't go to Aspen.

I just want to take a minute to recognize Shannon the sweet dental hygienist at the CHC dental clinic. She has been so welcoming to me since the first time I decided to try this dental clinic. I was very nervous because I had not been to the dentist for a while and she was so calming and understanding. She always remembers me and everything we talked about at my last visit (6 months ago). Need your scrip refilled you better call two weeks in advance! Waiting on lab results well that's another to 3 week wait so by then you may be already mourned over and sprouting daisies! Can't forget the 15 minute limit you are given with your PA and then you are forgotten or taken care of by a med assistant or possibly a nure. It would be nice to have the doctor/ PA for your whole visit. My last appt was for blood to be drawn to see if my vitamins were helping my anemia. Sat there for 2 1/2hrs. They closed at 6. Seen the nurse then the PA and all that needed was blood drawn to check my levels
