Services
This health center is a Health Center Program grantee under 42 U.S.C 254b, and a deemed Public Health Service employee under 42 U.S.C 233(g)-(n). This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health related claims, including medical malpractice claims, for itself and its covered individuals.
Citizens of Lake County for Health Care, Inc is committed to HRSA compiance, quality improvement, and transparency of our quality metrics. CLCHC is striving to provide health care to Lake County, Lauderdale County and the surrounding counties for hope of better health for all residents. Our Outreach and Enrollment Counselor can assist with life changes through out the year for the Marketplace insurances and can assist with enrollment in TennCare.
Citizens of Lake County for Health Care, Inc. has three clinics, one of which is located in Tiptonville, TN, Lake County Primary Care, which began in 1979; one located in Ridgely, TN, William B. Acree Clinic, which began in 1985; and another located in Ripley, TN, Lauderdale County Medical Clinic, 317 Cleveland St.
