Patient Forms Upon entering our office our receptionist Melissa or Alysia will greet you and welcome you as a member of our family. We will request you fill our patient forms located in the New Patient Center. The paperwork provides us with general information about yourself and your condition. X-Ray Studies Your specific condition may require us to take x-rays to get a full evaluation of a patient.

They are always very friendly, patient, accommodating. Always try to work with you, your schedule. Explain everything & answer any questions you have. Lend an ear whenever needed. Try to work with you with any financial situations you may have. You go in to the office & they treat you like you’re family (the good side)! They are THE BEST!!

Came in with a spine that was in worse shape than I thought. Dr. O’Dell took X-rays and then went over them thoroughly with me and came up with a game-plan on how we were going to tackle it. Between Dr. O’Dell coming up with the game-plan and Dr. Scott adjusting me 3x a week it has already made such a difference! My headaches are gone and the pain in my back is getting better! Incredibly glad I found O’Dell chiropractic!
