Sugar Creek is a privately owned senior care center and skilled nursing facility that strives to meet the needs of each Resident while maintaining their privacy, dignity and integrity. At Sugar Creek Nursing and Rehab, we understand that no one looks forward to entering a skilled nursing facility, even if it is only for a short term stay. The role of our Admissions Department is to assist families who need to find immediate placement for a loved one. Often, a Discharge Planner or Social Worker at a hospital informs a patient's primary contact of the need for short term rehabilitation or long term care placement.
Sugar Creek Nursing and Rehab offers a full-time, therapy and rehabilitation team. We provide physical, occupational, and speech therapy. Our goal is to assist each individual to achieve their highest possible level of function and independence. My wife's grandmother lived here for 3 years before she passed. Look, no nursing home is great. It's a business. But, Kindred wouldn't allow any luxury at all, strict everything. This place at least gave her Diet Pepsi and palatable food and treated her like a human being. Could it be better? Yeah, but to do that, these places need to be non-profit.

These poor poor people...where do I start? The I dont give a care attitude of the nursing staff? The lack of attendance to the patients...lack of personal hygiene care, or should I start with the smell of fecal material that smacks you in the face when you enter the front doors...these poor patients are improperly medicated, poorly cared for and are in my humble opinion being taken advantage of. Bottom line...please..if you want to give your loved ones any dignity at all...don't send them to this place.

I would not give this place any stars but it would not post without one. I visited on June 22 & 23, 2019 to see my sister. At the main door I went to the nearest nurses station and asked for my sister's room. I was told they had no one by the name I had given them. And I was directed to go to the recovery place in front of this building. My sister arrived there on June 11th so maybe the lady wasn't knowledgable of that. Another nurse heard the conversation and corrected the first and showed me to my sister's room. The stench of urine was in the air but I could not tell where it was coming from
