Come visit our financial advocate for assistance. 24-hour on-call coverage is available, and we serve seven counties surrounding Fort Dodge as well as Mason City and Dayton.

At Community Health Center of Fort Dodge, we have been serving members of our community and surrounding areas as a federally qualified health center since 2006. A group of local churches, physicians, agencies, and businesses identified the need for medical and dental assistance. Provides comprehensive primary health care to medically underserved populations, regardless of ability to pay.

Each of our locations offer additional outreach services to those in and around the local area. Nurse Practioner was very nice and thorough but it has been 3 weeks and I have never received my test results even though I have called to ask for them.

Staff is always friendly. They're generally pretty quick and knowledgeable. I've never had a bad experience here. Sure once or twice I had to wait a little longer than normal but still wasn't a big deal. The dentistry side also has always been great to me. Don't know what I'd have done without them. Thank you all.

THE SLOWEST medical service I have ever had, makes you pull up to get tested for corona, then a whole hour after my scheduled appointment time I am sitting here still waiting to see a physician and am told that I cannot leave until I do so, so you guys take over an hour for a 30 second test and now I have to be late to my SECOND job because you can’t schedule my appointment right or figure out how to work fast and efficiently? Ridiculous.

I must say, this place has done a complete 180 from what it used to be. The doctors are extremely nice, behavioral Health side is absolutely amazing and I would recommend them to anybody, and the dentist part of this community health center is absolutely phenomenal compared to what it used to be and especially for accepting title 19. They also have sliding scale fees for those who financially are struggling or do not have health insurance. I have been treated with the uttermost respect here. well done and props to you guys

Took over a year to get a billing issue fixed even after telling them multiple times about the problem. Didn't get fixed until I quit being polite and unfortunately was forced to write a nasty note to them. The person that finally responded was very nice and got it done.

Went in to get a tooth pulled. They started poking at it and yanking it around just for them to stop and tell me they don't have enough time because they had another patient coming in so they sent me out of there with a tooth that now feels like I have an exposed nerve.

This is the absolute worst health care facility I’ve ever been to in my life. The staff is rude. They cannot even schedule appointments properly. They are totally incompetent. This place is an embarrassment.
