Our skilled nurses are integral to the home health team. In most cases, the nurse will be the first to make the visit meeting with the patient and caregiver. On that initial assessment, the nurse will assess, evaluate, and determine the needs for home health. Once the start of care begins the home health nurse will continue to follow up with visits by assessing, teaching, implementing the plan of care, and providing skilled care.
Our goal of physical therapy is to increase the activities of daily living for our patients such as; grooming, bathing, walking, toileting and mobility to transfer. Since as we age our mobility can become limited, physical therapy for seniors can help them retain their independence by improving their overall health and mobility, as well as help manage a long-term illness or chronic pain from arthritis.
Occupational therapy is a treatment that focuses on improving life skills. Participating in an occupational therapy program for seniors usually takes place after an injury, illness, or a mental health condition. Very grateful for the compassionate care. I truly appreciate the training given to my mother's caregivers and myself by the Nurse Coordinator, Andrew. My mother suffers with dementia and it is our desire to provide at home care. The training we received made a remarkable difference. We noticed immediate results thanks to Andrew. Over the course of the last two years staff members, Mindy, Tito,Katelyn, Mila and Maria have all provided excellent care for my mother. Assured home health was recently hired to help my mother, who was in need of wound care, as well as post-surgery care. They are disorganized, didn't follow-through with what they said they would do, and required a ton of phone management, on my part, to make anything happen. On the up-side, the nurses were nice...when they showed-up. We have since switched to Signature Services (another Portland-area in-home health care company), and are receiving service that is much more professional.
