To improve the health of the population in Southwest Georgia by providing patient-centered, preventative, primary health, behavioral health, enabling and health education services, with an emphasis on the medically uninsured and underserved, regardless of the patient's or the family's ability to pay.

Primary Care of Southwest Georgia (PCSG) and its dedicated staff are here to provide your entire family with quality health care! We have six locations across southwest Georgia that service Early, Decatur, Thomas, and Brooks counties and all of the surrounding communities.
Services
We want to welcome you as a new patient to PCSG! In this section, we'll provide some information and details that will help you prepare for your first appointment. Then, just bring them with you for your first appointment. Many of our patients tell us they like being able to complete the forms at home!
A provider is always available by phone after hours for both adults and children. A PCSG Provider may be reached by calling the number of the office at which the patient is seen. All phones are forwarded to an answering service at the close of each business day, on weekends, and holidays. Our goal is to always provide access, and even expand access, to your provider so you always can reach PCSG when you need us most!
At PCSG, our main objective is to protect and care for our patients. Our Compliance Program ensures that we, as an organization, remain focused on what is ethical and fair for our patients, our providers, and staff members. PCSG's Compliance Team is dedicated to protect our patients, our providers, and staff members as we navigate through all aspects of compliance.
A medical home is an approach to providing total health care. With your medical home, you will join a team that includes health care professionals, trusted family members and -most importantly-YOU! The medical home is a way for you to be informed and involved in your health care decisions. If you need to see a specialist, your team can keep in touch with the specialist to make sure you get the care you need.
Primary Care needs documentation of proof of household income and the number of people living in the house. The Blakely Site partners with Spring Creek Health Cooperation. This place SUCKS! I called this morning and spoke with Yolanda and informed her that I had tested positive for COVID19 yesterday at Smith Northview in Valdosta and that my husband had been diagnosed the day before and i tried to call this office yesterday and the day before and I finally called Brooks County hospital and the lady that I spoke to… told me that this offices phones were down in Quitman. I called this morning to see if I could get either the B.A.M. infusion like my husband did or some Ivermectin. I called at 11:56 a.m. this morning and I told them I had my test done at Smith

Who ever sets up appointments made a mistake with my appointments causing me to have no shows on my records. I called the site manager who with another young lady quickly resolved issue

I never had a primary Dr. and with no issuance, they didn't care. They only wanted to help me get better. I would and have recommended this facility to anyone.

Went here when I got diabetes, and had no insurance. They took care of me. Prescribed me the meds I needed to live everyday with my diabetes. People who say they don't care are liars in my book. They care, maybe a little too much for some people's liking. I will always be grateful to the nurses & doctors at Primary Care of SW Ga.
