Services
Bornemann Senior Communities enriches the lives of local seniors and their families by providing exceptional and personalized care to everyone we meet. We help people recover from surgery, provide quality long-term care to those who need 24-hour monitoring, and create a beautiful environment for people to live and call home.
Our goal with rehabilitation is to get you home as quickly as possible. We know you want to get back to the life you love and we do everything possible to help you get there. If you've recently been released from the hospital, but aren't quite ready to return home, our highly skilled and compassionate nursing services will provide the bridge you need to return home safely.
At Bornemann Senior Communities, we recognize the important role our staff plays in delivering the best care and health services in the Green Bay area. That's why we reward them with highly competitive salaries and a comprehensive benefits package that includes health/dental insurance, generous paid time off and a 401(k) plan.
We understand that when you come to us for rehabilitative services, your first goal is to return home as quickly as possible, so you can get back to spending time doing the things you love. We are dedicated to helping you exceed that goal, and make sure you return home in the best possible state of health and well-being.
For patients who have been discharged from the hospital, but still need comprehensive care, our subacute nursing services provide a bridge between your hospital stay and returning home. Our skilled team is available 24/7 to help ensure a safe and comfortable stay while working to facilitate a return home.

This place is a terrible place to take a hospice patient. They changed their food contractor, food is terrible and portions the size a 2 yr old couldn't get full on, requested to change roommates, they didnt comply (roommate made accidents in his pants, taking them off leaving them on the floor, being disruptive in other ways), kitchen staff making fun of a bariatric patient. A few patients belonged in a mental facility instead of a nursing home. So much disregard for a hospice patient (and family) like everyday was their last with the least amount of respect. If it wasn't for Unity Hospice staff

Allegedly I would not recommend volunteering here. Allegedly I would not recommend living here. Allegedly I would not recommend a visit here. Allegedly I would not recommend seeking employment here because you allegedly may need to seek another place of employment soon after being hired. Allegedly I find the activities department and the administration to be allegedly dishonest and personally I allegedly would not even send a postal worker to this facility to deliver a letter.

My father stayed here and they were expensive for subpar barely legal care. Also they were super friendly as soon as they got his money. After he passed give me this jump through hoops to get a refund and continued saying the check was sent out. Once the check was finally received we check the postmark date and realize they've been lying to us.
