We provide high quality health care services in the communities of: Glenns Ferry, Grandview, and Mountain Home. Our medical and dental clinics are staffed with excellent health care professionals delivering high quality health care to all. With flexible hours and several community locations, we are available to provide excellent services for all the health needs of you and your family.
A valid email address is required to schedule online; otherwise call the clinic to schedule. Please have your drivers license and insurance cards ready before you begin the scheduling process. Skipping sections of the vaccine registration tool will cause delays on the day of your appointment. You may receive the Moderna or Pfizer vaccine depending on availability; we cannot guarantee which vaccine you will receive on the day of your appointment.
Does Desert Sage provide a sliding fee scale for those who qualify for one? Yes -Desert Sage does provide a sliding fee scale for those who qualify. A minimum payment is due at the time of check-in. Any additional charges are collected at check-out. If you require a payment arrangement, please contact Patient Accounts prior to your visit.
Desert Sage Health Center has established a program that reduces fees charged to patients based on their income and household size. To determine your discount, Desert Sage Health Center compares your income and household size to the "Poverty Levels" annually determined by the federal government. Patient fees are reduced in the form of a nominal charge or as a percentage of our standard fee.

Went to see Nicole. Within 30 min she tried pushing pills on me. AFTER I had told her that I once had a problem with them and preferred not to be on medication for my anxiety and depression. She didn’t care. My mother works for McKessen, so I know how this whole thing works. So if you want a pill pushing doctor. Go see her then. She ignored all of my other concerns such as my heart issue. When I didn’t take her up on her pill offer, I was dismissed. Won’t be back.

I just moved to Boise and got an infection. My boss told me to call Desert Sage because Mtn Home may be a drive but they have Denists that have a small case load. I was seen within a week. Given better meds and the extraction was not as painful as others in the past. They have a sliding cost so that was a godsend. My new insurance shows Desert Sage as out of network otherwise I would have been staying with them.Thank you Dr Juli.

Got all test covered.Wasted no time, wonderful bed side manners. Asked question I never thought about. Been going here 2 years. I HAVE always gotten in within a timely matter when I had no appt.

The worse experience I have never had in my life. I will not even take my cat to that dentist? provider because I love my cat. She broke my existing crown that I paid $1,800 in California. They blame me because supposedly I signed and I couldn't do anything about it. After almost six hours of pain to remove one tooth. There don't even know anything about dental work. I have been a dental technician since I was 18 years. I asked why they want me to sign and the assistant said to remove the tooth. If she is a dentist, I am an astronaut.

Chad Steele is the Superman of dental hygiene. He has years of experience in this field and it shows. He is gentle and thorough! He is fast and painless! Truly one of the best. I am so happy to have found such a great office and such a talented hygienist. I will be returning to him for all future cleanings.

If I could RATE w a million stars I would!! 5 is not enough for Dr. H! He is very sweet, kind, caring, and really takes his time listening to his Patients and asking what they think! Not like other Doctors and they tell you what you think! He’s so patient, and seems like every patient of his, is very important ! He is amazing and when he suggests something, like f you don’t agree you feel so comfortable in telling him NO and WHY and then there is always a compromise! I have never left unhappy after an appt and I would recommend anyone to go and see him! You will never want another Doctor!!

The team here has always taken great care of me. I get my teeth cleanings done every six months and the dentists give great advice on how to improve hygiene. My doctor has always been quick to hear my issues and refer me to the right specialist for the job. No one has ever made me feel silly or dumb for asking questions.
