Strong Family Health Center, formally Modoc Indian Health Project, is a product of the vision and dedication of Erin Forrest, the Project Director from 1968 until his retirement in 2002. Early in the 1960's, as a member on the State Commission Board representing Indian issues in California, Erin helped initiate important changes in services for Indians in California.
He helped secure and develop programs to improve, medical and dental services, nutrition and employment. His ideas are included in what is now the primary Indian Health Bill. In 1990, after a quality assurance study, Erin's program at Modoc Indian Health Project was selected by the Health and Human Services as the most cost-effective health delivery system in the United States.

Services
Strong Family Health Center is an Indian Health Services (I.H.S.) Contract Healthcare Services Organization. We serve any Native American/Alaskan Native residing within the county of Modoc (excluding anyone living on the Fort Bidwell Indian Reservation). Please reference eligibility under the tab policies.
Strong Family Health Center's Special Diabetes Program for Indians chooses a "Best Practice" provided by The IHS Division of Diabetes for each grant cycle period. The overall goal is to provide information and resources to strengthen clinical, public health, and community approaches to diabetes treatment and prevention in American Indians and Alaska Natives.
The PRC Program is integral to providing comprehensive health care services to American Indians and Alaska Natives (AI/AN) through contracted health services. The combination of an increasing AI/AN population, limited funding, medical inflation, and limited competitive pricing and options requires strict adherence to program guidelines to ensure the most effective use of PRC resources.
Our Social Services Program strives to provide quality and culturally appropriate Social work services to American Indians and Alaska Natives. The overall purpose is to help Indian clients maintain, regain, or enhance their level of social functioning when their ability to function is affected by actual or potential illness, disability, or injury.
SFHC nurtures excellence in holistic approaches that promote healthy lifestyles, families, and communities. This program address alcohol and substance abuse by improving access to behavioral health services and counseling services and rehabilitation treatment coordination.
