Acute Home Health Care

Licensed nurses assess and monitor the patient's health-care needs, educate patients and caregivers and collaborate with attending physicians to promote continuity of care.

Some examples of nursing care provided in the home: Medication management, wound care / ostomy care, post surgical care, bowel and bladder management, understanding a new disease process or diagnosis, urinary catheter insertion/maintenance, injections, infusion therapy, parenteral feedings and care.In-home care is one of the greatest long-term needs for seniors with chronic illness or for those who are unable to attend to themselves.

As long as the patient's meet the admission criteria, there is no distinction in eligibility for, or in the matter of admitting, participating and providing all patient's services, and benefits all persons, groups, or organizations who have the capability to either refer or recommend patient's for care and services to Acute Home Health Care, Inc. are advised to do so without regard to the person's race, color, gender, national origin, physical disability, religion, height, weight, marital status, political beliefs, financial status, or age.

Medicare, Medicaid and Commercial Insurance and Private Pay are all possible payment sources for home health care services. If one of the above services is providing care, then the patient may also receive Occupational Therapy. Medical Social Services, and Home Health Aide care as needed. Necessary medical supplies and durable medical equipment may also be convered.
