In 1910, Dr. Jennie Schofield and a group of community volunteers founded the "Wheel Chair Home" as a not-for-profit home for seniors in Western New York. Today, Schofield continues to provide high quality health care services in our rehab and nursing facility and community-based health care programs.
Services
Adult Day Health Care - Health care, social activities and support services during two sessions, six days a week. In-Home Care - Providing the care you or a loved one needs in the comfort, privacy and safety of your own home. Traumatic Brain Injury - Several programs including waiver services to address the cognitive, physical and emotional aspects of individuals with TBI.
If you or a loved one would like to remain independent at home, but need help with care, Schofield's award-winning Adult Day Health Care program can help. The Adult Day Health Care program enables someone to remain living in their community while they receive the health and rehabilitative services they need.
Whether you or a loved one are recovering from an acute illness or injury, living with a chronic disease, or just need a little help with daily tasks, Schofield Home Care can help. We bring Schofield's quality care right to your doorstep, providing the care you need in the comfort, privacy and safety of your own home.
Many individuals come to Schofield to receive short-term rehabilitation from a facility they can trust. Our interdisciplinary team develops a plan of care for each resident, allowing the individual to achieve their maximum functional status in the shortest amount of time. Our goal is to help you recover and return home or to a lower level of care.
Schofield's Mondello Wellness and Family Center is an extension of Schofield's mission, which is to provide the highest level of care for the physical, psychological and spiritual needs of those we serve. The Mondello Wellness and Family Center is located in Schofield Residence in the Town of Tonawanda.

Watch out they claim they are right and the patient is wrong. They ask the person they sent and what ever the employee said it's considered the truth and to bad for the patient the patient will never have a voice. Unless they are badly bruised or killed

They are terrible!!! Very inconsiderate, half the time the aide doesn’t show up and when she does, she plays on her phone most of the time. There is a list of things she is suppose to do hanging on the refrigerator and half the stuff never gets done.
