The Patient-Centered Medical Home (PCMH) is a model of care that emphasizes care coordination and communication to transform primary care into "what patients want it to be". NCQA PCMH Recognition is the most widely adopted model for transforming primary care practices into medical homes. Research confirms medical homes can lead to higher quality and lower costs, and can improve patients' and providers' experience of care.

I have been coming to Shasta Community Center for eight years now seeing Dr. McMullin who is one of the best doctors ever. His office staff are wonderful people and I have never had a bad experience. Dr. McMullin is one of the kindest, understanding person who really cares about all of his patients.

I am extremely happy with the personal attention given me over the last year, with very informed staff members guiding me on my decisions about matters regarding care of my heart, my arteries, my diet, and other various systems! They are very caring, thorough, and quite willing to spend quality time with me in helping me make detailed decisions regarding medications and other needed areas regarding my quality of life! I'm very, very happy to recommend this caring group of facilities!!!

I wanted to thank Diana in Pediatrics for going above and beyond. Even though it was not her department, she helped me time after time to get in contact with the Urgent Care department. Waiting on hold for urgent care to pick up for 35 - 40 minutes every day for the last four days to get through to the doctor who saw her for her ankle. Finally after days of waiting and calling, sweet Diana helping me again, she told me they ordered an ultrasound for my daughter. It would have been nice to get a call. Thank you for all your hard work Diana!

The location on Placer st is wonderful. The staff is amazing and my kids don't mind going there which says a lot. However, the location in Shasta Lake has made me feel very unwelcome and actually they refuse to see me. I need my medication for my thyroid terribly bad and for over a month now they have refused to see as a new patient. I have filled out all the paperwork and physically gone down there 3 times (not to mention the dozen times I have called them) to try and schedule an appointment and because my previous doctor wont send my records, they are refusing to see me. I'm actually at a loss
