Please note, if you provided us with the name of your primary care physician when you visited our office we have forwarded your information after each office visit. If you did not have a primary care physician at the time of your visit and would like your records, this will be accomplished through our online patient portal.

You will be sent an invitation to register for our online patient portal to the e-mail address you provide. If you do not have internet access, medical records inquires may be sent to Bone & Joint Medical Center, P O Box 1588, Adrian, MI 49221. Include a completed copy of "Authorization to use and disclose information form."
