Maplewood of Sauk Prairie has been caring for residents of Sauk Prairie and the surrounding community for almost 50 years. We are a locally owned, rehabilitation and health center on the campus of a thriving healthcare community. Our social services representatives specialize in assisting prospective residents and their family members with the transition to Maplewood. They will assess your loved one's situation, explain the admission and payment process, and review the alternatives for returning home. Following admission, our social workers continue to monitor the resident's well-being, serving as a liaison for the resident and family members.

My mom was at Maplewood for 3 weeks for rehab. I am so pleased with the care she received while there. When I asked her if she liked the nurses there. She replied " They were perfect ". I guess that says it all!

My mom was admitted for diabetes related problems. The staff noticed problems with my father while he was visiting. He has since been diagonsised with dementia. Thank you for caring for patients and family. Also, Staff would give us updates on mom all the time. They would stop and help her whenever we asked. Thank you for all you have done for mom! Everything was excellent!!

I am a retired farmer from 16 miles west of Sauk City. Last summer different scans indicated I had a suspicious appendix, possibly cancerous and that it should be removed. I had major surgery at the UW Hospital on December 1, 2017 removing the appendix which by that time had broken open and was releasing cancerous fluids into my abdominal region. I spent 12 days recovering at the UW Hospital and then was transferred to Maplewood on the 12th of December where I resided until January 4th in the rehabilitation unit receiving care and therapy. A few days before Xmas, 2015 I lost balance at home and fractured pinky finger.Then, early Xmas
day I fell and fractured my wrist. After that, it was decided I should have physical and occupational
therapy at Maplewood. The staff was very friendly and helpful to me and I think we became pretty good friends while I was there. The therapy did me a world of good. As usual, time away from home is good,
but there'should no place like home. Thank you, Maplewood!

My husband had young onset Alzheimer's and Lewy Body Dementia. He was at Maplewood during the last few months of his life and the care he received was amazing. The staff treated him so tenderly and with so much compassion it almost made me cry. It was very clear that everyone had been trained in dementia care, which is so important. I knew that he was in the right place when I heard laughter in the halls...if the staff is happy it's a very good sign. I would recommend Maplewood to anyone looking for a place for their loved one (food is good, too).

Hi. I'm Tom Schwartz. I live in Dane, WI. In February of 2015, I had both hips replaced at Sauk Prairie Hospital. I was sent to Maplewood on February 6th to begin my rehab. I can honestly say that I didn't know what to expect. The staff was outstanding! From the kitchen staff to the nurses to the therapists who helped me get my life back. I am now living a productive and pain free life. The therapists showed me great patience and kindness during rehab. My nurses were always there if I needed anything and they were soooo nice! The hospital suggested we go to Maplewood Nursing Home after my husbands knee surgery. We were impressed with the care he received.
The two weeks he was there, he received 38 hours of rehabilitation. The staff was very knowledgeable and helped him get where he wanted to be.
He was strong enough to go home and do for himself. Overall,everything was wonderful. Everyone was friendly and encouraging. We would recommend Maplewoods physical therapy to anyone who is need of physical therapy.

Wow. Just wow. This facility does not honestly report information, and uses bureaucratic red tape and misinformation to extend stays indefinitely. They do not feed the patients enough food, they are consistently understaffed and completely ignore their patients whenever possible, and worst of all, they feign ignorance when questioned about anything regarding their patients, their care, or the conditions. They do give misleading, inaccurate answers whenever possible, so if you like being misled, there's that. Best of all, they play the carrot-and-stick game, continually insisting that your loved

I had a knee revision surgery at SPHC on July 15, 2015 and had a couple of options after for help with rehabilitation. One was go home and have in-home rehab where the company would send a qualified person to my home to help me with my therapy a couple of times a week or go to a rehabilitation center. If I would have chosen in-home therapy that would have meant I would have had to stay home all the time while they were working with me. I would not have been allowed to go for car rides or to visit family/friends etc. I would have been only allowed to go to my Dr. appointments while in their care
