Community Alternatives
Community Alternatives first opened its doors in 2013 after Shawano County began to transition to Managed Care. Prior to this, Residential and Community Services were provided through Shawano County Department of Community Programs. Shawano County had set up Community Alternatives to purchase affordable housing for persons with disabilities as well as oversee the qualified staff that were employed through D.E.N.

Services. We had a wonderful group of people help with all this and we slowly began the transition in to the Community Alternatives of today. In 2015, Shawano County transitioned to offering FamilyCare. Community Alternatives currently are service providers for Lakeland Care, Inclusa, and IRIS. We also accept private pay.
Services
Community Alternatives currently supports individuals in 16 residential homes. Residential homes support anywhere from 1 to 4 members, and are generally supported 24 hours per day with in-company nurse oversight. We work hard to build homes in which the housemates have commonalities. We also work hard to match Team Member personalities with personalities of the Members in which they will be supporting.
Millions of aging Americans living in long-term care facilities face cognitive and physical difficulties and have left behind their familiar surroundings, familiar faces, and even their favorite music. Despite the best efforts of loved ones, their lives often lack meaning, spontaneity, choice, and reliable social interactions.
