Services
The mission of Acacia is to provide comfort, assurance and care to our patients and their families. Proactive in instituting and collaborating with various providers and experts in expanding programs to meet the dynamics of health care needs for the community we serve. The recognized leader in quality care and reflecting the diversity within the community we serve.
As we grow, our need for qualified, dedicated associates in each of these areas grows with us. While we appreciate your interest in joining our team, we advise that you not send letters and resumes to our office unless we post the position. Acacia is now accepting applications for RNs, LVNs and Certified Home Health/Hospice Aides.
Patient Protection Affordable Care Act provision dictates that physicians wishing to order home health care services for patients covered by Medicare must schedule a face-to-face encounter. These visits must occur 90 days prior to, or within 30 days of the start of a Medicare home health care episode, and that each visit must also be documented using the Face-to-Face Encounter Form.
The goal of Hospice is to provide care for people who are terminally ill and to manage their pain and other symptoms. Focus on the continuous, integrated health care process that a patient/family experience across all aspects of hospice care. Incorporate outcome-orientated quality assessment and performance improvement.
Often the need for hospice care is not obvious, or the caregiver is too close to the patient to effectively observe and evaluate small symptom changes that are not alarming by themselves, but when combined, they show that the patient is experiencing a decline in their overall health, their level of comfort, and their ability to function.

I had both my mother and mother in law with this company. They were once a good company. my mom started care with this company when Jackie was over her case. It was wonderful. Jackie was a get in dig in and get the job done type of person. High Caliber and always on top of things. She left and they brought someone else in, who just could not handle to the case load of keep up. They also stopped listening to the family. They wanted to give my mother two OT visits and no physical therapy. This company changed and only cares about $$$$, not who I want on my family members care. They barely send wouund

The absolute worst. Make appointments, then not show up. When they did show up all they did was take my blood pressure, then leave. Bottom of the barrel and not professional at all.

Great staff. My father's nurse is insane. She is a true angel. Her name is Behi. An angel nurse with a huge heart. I love my parents and give all my love to them. However, nursing and giving care is stressful and requires patience, training and knowledge. Nurse Behi did so much for my dad. She also taught me how to care for him with more detail to get the bewt possible result. Also Mr. Alex and his s staff at Acacia made certain that we as a family understood all our options and made every attempt to ensure my father's full recovery. This operation is first class. Thank you again! God bless 🙏🏼✌🏼
