At Family Health and Sports Medicine, our board-certified Family Physicians are devoted to the health care of patients of all ages. Dr's. Family Health and Sports Medicine was founded in 1983 by Dr. Al Puerini on Park Avenue, Cranston, RI. As an avid runner, Dr. Puerini's goal was to provide the community with high quality primary care and to promote a healthy lifestyle through exercise and nutrition. Sharing the same passion for helping families live healthier, Dr. Mark Rosenberg joined FHSM in 2003 in its Reservoir Avenue location.
Sports Medicine is not only the treatment of injury during competition, it encompasses a wide variety of health issues related to an active lifestyle. At FHSM, we provide high quality care not only for bumps and sprains but also for other serious medical issues that can impact an athletes performance including eating disorders, sports nutrition, and concussion management.
Family Health and Sports Medicine (FHSM) has been recognized by the National Committee on Quality Assurance (NCQA), after much preparation on our part, as a Patient Centered Medical Home (PCMH). What is a Patient-Centered Medical Home? It is team-based health care led by a physician or group of physicians.
The best way is to request a medication refill directly from your pharmacy. If you do not have any refills left, your pharmacy will automatically send us a request. Please call at least 5 days before you will run out of medicine to allow for days your doctor may not be in the office. Controlled substances will not be refilled when the office is closed including nights, weekends, and holidays.
Full payment of your co-pay or deductible is expected at the time of your appointment. Payment of your co-pay at the time of your visit is an obligation you have with your insurance company. It also helps us to continue to provide the excellent medical care you expect from us. We accept cash, check or credit card.

DO NOT GO HERE!!!! The office staff is rude and dismissive. I used to see Dr. Jeffrey Wilson and changed to another doctor at another office due to the lack of assistance and compassion from the office as a whole. They send you threatening letters after a you visit an urgent care or emergency room saying that you should have called them first but when you do call them they basically act like they have no responsibility in helping you with your matter at hand. My husband was a patient of doctor Wilson, but today we will be working on changing him to another doctor at another practice as when he

My insurance automatically assigned me to dr. Rosenberg. I called a month ago to see if they were accepting new patients and got a very rude young lady saying they were never ever accepting new patients again. I’m pretty sure there’s a nicer way to communicate that, or add to your website that you’re not accepting new patients instead of “make an appointment”. Maybe the doctors are great, but if that’s how they receive inquiries...! I tried to let it go but now I’m dealing with way nicer team of front desk personnel and I had to share.
