Adhesa Plate is certified for Authorized Label Supplier Program of UL labels and nameplates PGAA file # LP1964. Adhesa Plate is also certified for UL Marking & Labeling Systems - Component PGDQ2 (UL969) File # MH12483. Adhesa Plate UL 969 products are certified for indoor and outdoor use and tested for Canadian standard cUL.
Adhesa Plate's customers can choose from sequential numbers, static barcodes, random serial numbers, or any specified data to be printed with their label design. We offers wide variety of material choices and 3M adhesive options for all of our barcode & QR code labels which makes them suitable for application on just about any surface.
Custom dome or domed urethane coated labels are made from white, clear, chrome silver and brushed silver polyester label materials. A.060" durable, clear, non-yellowing urethane coating adds a high end 3-D look to "Identify your Product". Outdoor durable which can self-repair any dents that occur. Available with an LSE adhesive for powder coated surfaces.

They promised:
will have a quote to you within 6 hours of our business hours, within 12 hours if the quote request is received while we are closed, except weekends which will be the next business day. Our hours are 7:30 am – 4:30 pm, Monday – Friday.
Initial order 12/9/2015; never heard back
emailed 12-23-2015; never heard back
Called 1-4-2016; they said they are behind, Then had the nerve to hang up on me before complain about being short handed.
