Dr. William Rice opened his practice, Evans Chiropractic Health Center, in 2005. The practice is off the corner of Cox Road and Belair Road in the Evans/Martinez area. As Office Manager, I am here to assist patients with all aspects of our office. I love interacting with our patients and hearing about their life events.
On your first visit you will be asked to complete a "Confidential Patient Information" questionnaire before Dr. Rice sees you. Some of this information will involve your past medical history. These questions are all necessary because they supply Dr. Rice with essential information about you. It is also important that we know the names and dosages of any medications you have been using or taking.
Most insurance companies cover Chiropractic. Dr. Rice is a participating provider for most of the major insurance companies. If you have insurance, you must bring your insurance card or plan information with you. We will take assignment for the portion that the insurance covers. You will be responsible for your deductible and the portion that your insurance does not cover, at the time of services.
