Carmen graduated in 1986 with an Associate's Degree in Nursing from Mario Munoz Monroe in Havana, Cuba and began her career at Pity Fajardo Maternity Hospital in the Newborn care unit, Emergency Room department and then Labor and Delivery room. She received her BSN from the Superior Sciences Medical School of Havana in 1996.
PHS' vision is to keep our patients home and out of the hospital. We are Medicare/Medicaid certified and licensed in Districts 10 and 11 and provide a complete range of therapeutic home health care services. Care is provided by licensed, insured staff who have passed an extensive background screening to ensure you or your loved one receives the attention and respect they deserve.
The goal of skilled nursing care is to work closely with the physician to improve the patient's condition. Our highly trained, experienced Registered Nurses and Licensed Practical Nurses monitor acute and chronic conditions, provide skilled care and instruct on medication and disease management.
Preventing re-hospitalization and leaving the patient with the knowledge and confidence to be able to care for themselves safely in the comfort of their own home, is the objective of our home health team.PHS provided very good care, the staff was on time, and spent time explaining things to me. PHS provides Infusion Therapy to patients in their home, allowing them to avoid a lengthy stay in a hospital or a skilled nursing facility. Infusion Therapy is provided with a core staff of highly qualified, infusion RN's who provide safe, quality care while communicating with your doctor and monitoring the effectiveness of your treatment.
