We can also mail forms to you if you'd prefer. Please call our office to make arrangements. If you are unable to do so, please arrive 15 minutes prior to your appointment to fill out the necessary paperwork. Here you can create an account to upload all your forms and keep up-to-date records for your entire dental history.

I have been a patient @ New Richmond Dental for a long time and I find Dr. Tietz to be friendly, patient and helpful, along with being an excellent Dentist. The STAFF and Hygienists are professional and caring individuals who make me feel welcome and important and I am pleased and grateful for each one of them. They have changed my 'fear of Dentists' into a rewarding experience.

What a fantastic experience!! As a brand new patient and a person with a severe phobia of dentists, I couldn’t be happier with my visit today. Dr. McFarland, Tiffany, and Kiersten were phenomenal. Every one of them was so kind and made me feel completely at ease. They explained everything so I understood and made sure I was comfortable during the entire exam and cleaning.
From the person who made my initial appointment to the person who made my follow up appointments and explained insurance coverage today, all of you were so kind and helpful. So happy the first dental office I called never called
