Telemonitoring services for diabetes and hypertension require you to meet certain qualifying criteria and may require an authorization form from your doctor. If you have either of these conditions and are interested in receiving telemonitoring services, you will need to discuss these qualifying criteria first.
Per the Texas Medicaid Provider Procedures Manual, telemedicine and telehealth services do not require pre-authorization in most cases. However, to qualify for telemonitoring, the service offered through Criterion, patients must exhibit specific risk factors to be eligible for coverage.
