The nurses, aides, and administration have all been very attentive and caring to my mother. Mom has had so many team members that have gone above and beyond to ensure she is happy. Not all of our jobs require licensure or certification. We have positions in dietary, housekeeping, laundry, maintenance and clerical, as well as registered nurses, licensed practical nurses, certified nursing assistants and medication technicians.

The amenities we provide are designed to make your stay both relaxing and convenient, allowing you to focus on improving your health and well-being.
We recommend that you visit the facility prior to admission, to assure both the potential resident and family that Rainbow Rehabilitation & Healthcare Center is the place to choose for your healthcare needs, however, it is not required. We would be happy to schedule a visit for you.
You can also ask your discharge planner or case manager at the hospital to contact Rainbow Rehabilitation & Healthcare Center and we will make all the arrangements while keeping you informed.We also have a liaison who can come visit you in the hospital. She will meet with you and your care team to ensure a smooth transition to our center.

This place is awful please if you have a loved one in need of rehabilitation services or nursing home care do not I repeat do not use this place. I placed my mother here thinking that she would get the care needed for her to heal and get back to her life after an amputation and all HELL. The staff is discourteous and unprofessional. They do not respond to emails at all. The social worker was very short and refused to answer phone calls and emails left. Upon answering she made me aware that she doesn't respond to emails. My mother is an amputee and needed assistance with being changed, and maneuvering

If I could give a zero rating I would. The care that my sister received while in the care of this facility was awful. No one should receive this type of treatment. Yes it's beautiful but it smells like urine up and down the halls. Sad to say my sister became septic and died. Don't believe the hype about this facility. If you love your love one, Do Not allow them to go here. My heart is still broken.......Please check your family member for bedsores!!!!!!

I worked at Rainbow Health and Rehab for over two years. Kim was an amazing boss. She was always fair to all the employees, she expects people to do their jobs and take care of the patients. Julie was also great to work for. I left because I felt it was my time and I now regret it because that was one of the best places I worked for! They always were kind and friendly towards me. I would have put my mother there if she was still alive when it was her time to go into a nursing home. The CNA’s make sure they take care of all the patients no matter how they treat the cna’s. We all know that sometimes

What other nursing & rehab center has had TWO DEFICIENCY FREE SURVEYS IN TWO YEARS?? Not only a beautiful building and grounds, but warm, inviting and homelike environment for the residents and family there. What I love about it the most is the positive and caring attitude of the staff and leadership there! Always smiling, and greeted with hospitality the minute you arrive. The food is delicious, and the rooms are spacious and each has own bathroom. I really liked the theater room where residents can watch movies or visit too!

When you have management in positions that nobody respects you have a facility that is running in chaos (Respect is a lot more than a smile in your face). You have CNA's taking pictures with cell phones of residents to report incidents that's a Hippa violation..I have worked here as an employee for a year they are terrible for their census CNAs go home from left to right so your family members dont get the proper care. you have 30 patients to one CNA on night shift. The staffing doesn't know what she's doing. And personally she is in no position to be the staffing she is not a leader. And all management

This facility doesnt smell bad which I felt was a huge deal until I found out that was the only thing good about this place!I wish I could tell the whole story of my experience here, the space is not near enough! I took my mom here for some minor physical rehab and it was the worst experience of our lives! Do NOT leave your loved one here! I wouldn’t leave a stray animal here for treatment!! They do NOT have any regard that you are family, POA or responsible party, do NOT expect to be informed about the care of your loved one. You can expect them to ignore you and if you dare to get emotional

I have know several people who have had rehab at this facility. From strokes to a hip replacement. The current staff under the new administrator Julie is great. She seems to have whipped the building into shape. I still remember when she was over the nursing department and very surprised to see her come back as the administrator. I will continue to recommend this facility to my family, friends and church members.

I would rate this center higher if they had enough staff! The staff that I have met, have been overworked and stretched too thin to meet the needs of the patients. However, these staff have been polite, kind, and helpful. Management needs to do something about this matter! They need to hire enough people to meet the needs of the patients and provide support to their existing staff!! I waited for almost 2 hours this weekend on someone to answer the call button. I finally went out in the halls and found staff to help me with my mother. Also, my mother's room has not been cleaned in weeks! The bathroom

This facility is very beautiful on the outside & inside. Rainbow is one of the newer nursing homes along with Christian Care Ctr. I’m shocked at the negative reviews & am not trying to take away from them but I do know they have a new Administrator and she is the best. Ms. Julie Cottrell is very loving, smart, caring, professional & gets the job done no matter how big or small. She cares for the residents, their love ones and the staff; in that order. With her coming on board I’m confident there will be better reviews because she is one of the top 5 Administrators in Shelby County. Good luck
