Services
Our mission drives the work that we do. The mission of CenterPlace Health is to provide efficient and effective delivery of quality, affordable health services to a diverse population in a compassionate and caring manner. CenterPlace Health is a Federally Qualified Health Center providing high-quality, low-cost health care to thousands of people at five locations in Sarasota County.
As a new patient, you will want to provide the health center with a detailed and honest medical history of your entire family. It's good practice to arrive at least 15 minutes prior to your scheduled appointment. Be sure to keep all scheduled appointments with your provider. It's also important to give an update of any changes in your health and report hospitalizations during each visit.
To see if you qualify for the sliding fee discount program, use the chart below to determine your income level based on your annual household income and family size. Individuals of a household both traditional and non-traditional are considered family. Families are tied together financially and CenterPlace uses Federal tax dependency to determine household/family.
CenterPlace Health is committed to serving all patients, regardless of their ability to pay. Although we do accept many healthcare insurance plans, patients who are not covered by insurance will not be turned away. Many patients are surprised to learn that they are eligible for low-cost healthcare programs.
Florida law requires that your health care provider or health care facility recognize your rights while you are receiving medical care and that you respect the health care provider's or health care facility's right to expect certain behavior on the part of patients. You may request a copy of the full text of this law from your health care provider or health care facility.

Dr. Meyer is the best doctor my husband and I have ever seen. She is very thorough, listens to what you have to say, and has amazing ways to make you feel like you are at home at that office. All the ladies there have been seeing our family for months now and they all greet us and laugh with us every time we go there. We get great front desk service as well, very friendly, very understanding, and fast. Thank you for being the best !

I took my 2 year old in today, Nov 2, 2021. He was seen by Dominique Meyer, NP. The entire staff from the front desk on back were terrific! This was our first visit to this location. We usually go to the Sarasota location, yet, they didn’t have an appointment available to be seen. They were so terrific that I can’t help but think maybe we should change locations, even though we love the other location. Everyone at the Venice location was exemplary. They were superb! Thank you for fitting into your already busy schedule.
