We understand that your time is valuable. This will allow you to fill out at home and bring to you appointment. On your first visit, you will turn in or fill out the new patient paperwork. Please bring your photo id and insurance card with you. It is perfectly fine, if you do not have insurance or you just may not have a card.

Highly Recommend to Anyone & Everyone who wants to be treated like a King/Queen when they are under his care! He has the Best Personality which includes all the way from being very gentle with your mouth, to letting you have an opinion on the color of your own tooth. I went in to his office for the first time around May 2020 to get a temporary front tooth Cap put on & get an impression of my mouth so he could send it off to the lab for a New Permanent one in replacement of the temporary one I had barely still on & was time for it to be done anyway. But, I couldn't believe his "Bedside Manner".
