Our newly remodeled facility includes a large rehabilitation gym with state of the art equipment to help accelerate the recovery process of our residents. In addition to our skilled nursing and rehabilitation services, we offer therapeutic recreation and social activities.

From intensive short term rehabilitation therapy to longer-term restorative care, our Nursing and Rehabilitation Center provide a full range of services to treat each of our residents.
Our experienced rehabilitation team is committed to providing outstanding results and one on one quality of care. Residents are empowered through personalized programs to reach goals of increased independence and ability. Resident families are also encouraged to participate in therapy sessions and are provided caregiver education and hands on training.

Words cannot express the gratitude our family feels towards Country Villa Bay Vista. Our mother was treated with dignity, respect, love and kindness during her 5 month stay.
The decision to move her into a skilled nursing facility was a difficult one to make. Knowing our mother was in such kind hands brought a sense of peace to our entire family. Graciela received the best care possible at this facility.

Im a man that knows quality service and sincere human kindness that comes from the heart this place is that and much much more, their level of professionalism and attention to detail helped me to relax and feel right at home and for covid 19 forget about it!! And its no wonder that they have not had not one single case at this facility from the physicians to the nurses and even the janitorial workers when they came in my room they were always fully robbed up with face shields and masks. Im telling you I highly recommend this place and if one of my family members ever need this kind of service I

BEWARE! I couldn’t give them 0 stars. My Father was here for 2 weeks in April 2020. They overmedicated him, despite paperwork and constant pleading with them. He ended up in the ER. After 3 days in the hospital, ALL of his personal belongings ($250+) were flat out stolen. Nobody could give us answers. Just BEWARE!!!

My dad was there for about a week. While there, to the best of my knowledge, he received good treatment. His physical therapist, Jennifer, was really nice and answered all my questions. She also explained what was being done and why.
The true angel of that place is May Rosas. She kept me abreast of everything going on with my dad. She is a kind human being. We need more people in the world like her.
Putting my dad in a rehab center was nerve-wracking for me. Ms. Rosas told me she'd look after my dad's well-being and she kept her word.
My dad has since passed, but it had nothing to do with Country

My dad stayed here for about 2 weeks. The first conversation with their team was pleasant. Diana listened to our situation and immediately got the ball rolling. She then transitioned us to Christina Brown our God sent angel. Our situation was a little complicated but she went above and beyond to make sure my dads transition went smooth. She is very knowledgeable and gave us advice and referrals to other community resources. She also communicated very well. She would send me text messages of statuses on everything through admission. When my grandfather needed extensive rehab after suffering a stroke, we toured many facilities in the Long Beach area. We are and remain extremely impressed at the dedication of the people working here. The rooms are nicely equipped with everything he needs and the therapists and social workers are simply amazing. He also has special dietary needs which they were willing to work with and if you know him, he doesn't like to miss a meal! He's pretty finicky and bit of a curmudgeon, but he really likes the place. Wonderful nurses and a very caring staff. That's all I can say about this place that's good. The facilities are run down and dumpy. Long Term patients are innermixed with physical rehab patients. Patients line the hall ways moaning. The physical rehab dept is very small with limited resources. The PT staff is wonderful! The staff keeps this place going. It's too bad the administration doesn't keep up the facilities. I transferred my wife out after 1 week because of the run down conditions. We found a nicer place for her to finish her physical rehab.

Today i have been very disappointed with this facility my aunt was here for four months and i have just had a enough with the unprofessional director and staff my aunt was a patient there and every time i entered the facility it smelled bad my aunt have got a big pressure wound that was big where your hand could fit in it i was told it get worse before better , its all about money with this facility they not doing a good job of helping the elder she would have dry skin, dehydrated UTI infections ,, the place is not clean they dont help the CNA helpers at all with more employee help the nurses need
