CCHC foster Collaboration across the Safety-Net Delivery System. We facilitate the collaboration among our clinic members to address their needs in order to better serve the patients. By developing strategic partnerships and coordinating with other services, we advocate for a stronger primary and specialty care delivery network.

We strengthen Community Clinics by facilitating a collective learning community through peer sharing, networking, and by providing direct benefits to member clinics and their patients. Comprehensive website, social media, and phone referral services connect community members with Oregon Health Plan Enrollment support, community health clinics, and other social service supports.
Services
This funding will be very helpful in assisting us to reach our current health equity goals with our member clinics. We would like to thank the foundation for their commitment to ensure quality healthcare for all. Found in 1985, CCHC continues to advocate for Safety-nets clinics and serve vulnerable population, thanks to our key benefactors and donors.
CCHC provides internship opportunities to college and graduate students interested in community health programs and outreach, nonprofit administration, marketing, social media, events coordination, public health research, grant writing and fundraising. Get school credit and mentoring; stipends available for some positions and projects.

Advocating for smooth transition for safety-net healthcare delivery system and continue access for critical health services when FamilyCare closed.
CCHC Enrollment work consists of helping clients with the Oregon Health Plan (Medicaid) off-site at many different events throughout the Portland Metro Region in partnership with the CareOregon Connect to Care Team. We attend an average of 20 events each month, resulting in around 1,400 client interactions per year, roughly 600 enrollments and over 100 address changes last year.
CCHC's health equity work is all about building a cohort of peers to learn together and facilitating monthly and bi-monthly equity trainings for our clinic members, we come together as a coalition to work towards providing care that is responsive to the languages and cultures of community members.

Very helpful! Walked me through the whole application process while applying with me. Also were able to recommend the best plan based on my circumstances. Were available to answer any questions I had regarding benefits. Also helped me when my application took longer to process than expected.
