Granite Creek Health and Rehabilitation Center is a modern, state-of-the-art skilled nursing facility, located in central Arizona in the historic town of Prescott. We are just east of Prescott High School on Scott Drive next to the senior living campus of Las Fuentes Resort Village. We feature 128 beds and beautiful spacious rooms in either private or semi-private settings.

This place was strange from the get-go. One, they did not have a nurse on duty at night. I found that out when I had a migraine attack and found myself unable to get so much as a Tylenol for pain relief. Two, there is nothing but a bed sheet separating the men from the women showers. Three, they needed to fire the cook yesterday.

This place should not be open. My Dad was sent here on 2 liters of oxygen after fighting covid and pneumonia for 10 days in the hospital. He was supposed to get PT everyday to rebuild his strength and recondition his body. He was put in diapers the next day for some unknown reason, my dad does not require diapers, his TV didn't work, didn't even come on. So all he did was lay in bed and get weak. We complained 3 times about his TV and nothing was done. And now my Dad is back in the hospital on 18 liters of oxygen. And we found out he's only had PT once in the week and a half he's been there and

I have been in granite Creek on more than one occasion. I got substandard care. I have been in for ivy antibiotic therapy due to bad infections. And there was a time when I did not get my IV because there was no one to give it to me. Also I needed my leg cuz I had wounds on my leg that were so bad they were almost half the amputate. They were ordered by my doctor to change my dressings every 4 hours. They told me they could not do it and they put a flimsy wrap on my leg on Friday and by Sunday I had a bad infection had to be taking the hospital in an ambulance. My roommate went in after Covid complications. She’s diabetic, anemic, early onset dementia, and signs of sundowners that worsens if not kept on a proper diet or regulated on medications. She isn’t her own medical power of attorney — my other roommate, her daughter, is. This facility did not keep up her diabetic diet, allowed her to not eat if she didn’t want to (which was a lot), did not give her her iron or diabetes medication, and gave her narcotics for pain without informing her medical power of attorney. My friend was sent to this facility about 2 weeks ago. She arrived at 8pm. She told staff she was incontinent. she was told she would have to wait. She waited for hours and hours. She told them she had lots of meds to take. They didn't arrive w/her. She told dthem she was on 5L of O2. They gave her a portable O2 which ran out in the night and was not replaced. She told them she was diabetic. They did not take her BG til next day. She was on prednisone. They did not give her that right away either. Her medical condition was not addressed til LATE the. next day.

The culture at GCHR is one that is very patient/resident centered. The concern and genuine care that the employees exhibit in every aspect of their individual positions, is supremely customer centered. Each staff member that I have encountered has a strong team mentality to do what is best for those in their care. I highly recommend this facility as a health care professional. I equate my experience here, as compared to the last facility I was employed, as going from a Motel 6 to a Hilton; and, I mean this wholeheartedly.

Because there was no other facility in town available, my sister was sent to Granite Creek. If you read Arizona news, you'll understand why I was not happy with this placement due to their many violations and other issues. The place seemed okay when I visited, however, the management and organization of the place is woefully substandard. Even though I was clearly named as the legally responsible party (by court order - documents I sent them numerous times) they kept "losing" that information. It was next to impossible to get someone on the phone -- ever. And meet in person? Forget it. Except for
