The focus areas were selected after an extensive analysis of health and population data. While the HealthierJC key coalition members will lead the effort to create awareness and coordinate implementation of the selected action steps, community organizations will lead the efforts on the ground. Enacting positive change is a community-driven effort.

The Partnership for a Healthier JC team believes that reducing these health disparities and improving health equity requires collaboration.
Services
Healthier JC was established in 2014 and began as a partnership, development, and branding office of the Jersey City Department of Health and Human Services. That year the Healthier JC office applied for a Building a Culture of Health grant from the Robert Wood Johnson Foundation (RWJF) and New Jersey Health Initiatives.
All too often, great organizations are doing great things in silos. In the health arena, service providers are often overwhelmed by meeting the needs in our community with little funding and scarce resources to promote important initiatives.

Upon award of the Culture of Health Grant from the Robert Wood Johnson Foundation/NJHI, the Core Team of the Partnership for a Healthier JC set out to use the funding in a way that would provide needed support to our amazing health providers, social service agencies and other health champions in our community.First, we researched our top urgent health needs and developed an action plan to address them.
As always, our Online Help Center is available 24/7 for any questions, service requests, or suggestions you may have. Building a Culture of Health requires that we understand our population and its unique features. In an effort to understand who is using our site to get access to health opportunities, we ask that you consider completing this optional portion of our questionnaire.
Increase health education & improve management of chronic illnesses through expanded and meaningful partnerships with stakeholders city-wide. The demand for information that can help individuals make more informed decisions about the various factors that affect their health will only increase as nutritional, clinical, and socio-economic options multiply.
Increase use of Primary Care Providers, Improve Coordination of Clinical Care, Expand Mental Health Access and Offer Suicide Prevention & Intervention Training to Providers. Demand for primary care services is projected to increase through 2020, largely because of aging and population growth and, to a much lesser extent, from expanded insurance coverage as the Affordable Care Act is fully implemented.
